Health Care, Jakarta - The Indonesian artist couple, Ringgo Agus Rahman and Sabai Morscheck are waiting for the birth of their second child. In 2016, Sabai gave birth to her first child, Bjorka, through a cesarean delivery process. Through his personal Instagram social media account, Ringgo expressed Sabai's desire to give birth to a second child through normal delivery.
Well, many mothers who have had a cesarean section in their first pregnancy and are interested in having a normal delivery for their next birth. Actually, can you or not, huh?
It should be noted that vaginal birth after cesarean section (VBAC) is considered a safe choice for many mothers and their babies. Doing normal childbirth makes mothers come home early from the hospital and was able to recover more quickly. Factors of the type of incision in the uterus and factors of medical history also determine whether the mother can undergo VBAC or not.
Potential Success of Normal Delivery After C-section
Many pregnant women experience VBAC without any problems. VBAC is a very safe option for women who have had a previous cesarean section and have been identified as being low risk. For women who have been properly screened and considered capable for VBAC, the success rate is between 60 and 80 percent.
There are several reasons why a mother may need to try VBAC. Some mothers may just want to have a normal delivery. Meanwhile, other mothers choose for the following medical reasons:
- Avoiding delivery by caesarean section, which has congenital risks such as newborn infection, postpartum bleeding, and anesthesia-related complications.
- Reduces the chance of blood transfusions to lower the risk of blood-borne diseases.
- Reducing the length of stay in the hospital.
- Shorter recovery time.
Overall it was found that repeat caesarean section in women had a greater risk of death than VBAC. This means that VBAC can be a safe choice. Well, mothers can also discuss this with an obstetrician during routine pregnancy checks at the hospital if you still have questions.
VBAC Risks to Consider
While there are benefits to having a normal delivery after cesarean section, there are also some risks that may be faced. VBAC can cause dangerous rupture of the uterus or tearing of the uterine muscle caused by a previous caesarean section scar. Uterine rupture can be dangerous for the baby and may also threaten the life of the mother.
If uterine rupture causes excessive bleeding, the mother may have to undergo a blood transfusion or hysterectomy immediately. However, cases of uterine rupture are still low, less than 1 percent, and are at greater risk for women requiring artificial induction of labor.
In addition, there is a greater risk of infection for both mother and baby in the trial case of vaginal delivery after cesarean section. If the mother is going to undergo VBAC, choose to give birth in a hospital with facilities equipped to perform an emergency caesarean section, if needed.
Preparation Before Doing VBAC
If the mother had a cesarean section in the previous pregnancy and wants to have a normal delivery in her next pregnancy, it is best to start talking about VBAC from the first prenatal visit.
Discuss the mother's concerns and expectations with the obstetrician or midwife. Ensure that your doctor or midwife has a complete medical history, including records of previous cesarean sections and other uterine procedures.
The doctor or midwife may use the mother's medical history to calculate the likelihood of doing VBAC. Plan to deliver the baby in a facility equipped to handle an emergency caesarean section. Also discuss the risks and benefits of VBAC during pregnancy, especially if certain risk factors have not yet emerged.
It is also necessary for mothers to know that if the initial delivery is done by caesarean section, then the next pregnancy should be postponed at least 2 years. This distance is also recommended for cesarean and normal deliveries. If pregnancy occurs less than 2 years apart, there may be a health risk. One of the most fatal risks is uterine rupture.
If the mother chooses to have VBAC at delivery, she will follow the same process used for normal delivery. The obstetrician will recommend continuous monitoring of the baby's heart rate and prepare for a caesarean section if needed.
